Eligibility Requirements:

Promotion is available to all newly joining IDOC Alliance Members, who have not received this promotion for a previous IDOC Alliance Membership of any type. Enrolling IDOC Service Clients are not eligible for this promotion. If an IDOC Service Client enrolls in IDOC Alliance Membership while the promotion is still in effect, this now newly enrolled Alliance Member is eligible for the promotion barring any or all previously stated restriction(s) were not met.

Eligible recipients who join IDOC Membership with Essential Vendor Programs will earn a $540 promotional payment; eligible recipients who join IDOC Membership with Select Vendor Programs will earn a $840 promotional payment.

Eligibility requirements for promotional payment:

1. New Member must enroll in IDOC’s CooperVision vendor program and (1) other eligible vendor within the first 30 days of membership join date
   1. Additional vendors to enroll with include: Alcon,  Carl Zeiss Vision, Three Rivers Lab, Charmant, Etnia Barcelona, and MODO.
2. Rebate will be issued as an IDOC rebate during the applicable regular quarterly check run. Member must make their first three months of Membership payments and be a member in good standing at time of payment to be eligible.
3. One-time promotion is available to new IDOC members only.
